Transitions for Healthy Ageing
What if later‑life transitions weren’t challenges to overcome, but opportunities to shape? 
The PATH – Planning Active Transitions for Healthy Ageing is a 30 month Erasmus+ Cooperation Partnership in Adult Education (KA220-ADU) project was created around this very question. Our mission is to strengthen the support available to people aged 55+ as they navigate key turning points in life. Through a partnership spanning five countries, we work to ensure that these transitions become moments of growth, learning, and renewed purpose.
Across Europe, millions approach retirement with a mix of curiosity, uncertainty, ambition — and sometimes hesitation. We aim to transform this moment into a supported, meaningful, and confident transition.
We believe ageing is about change, not decline. That’s why our project supports people aged 55+ in staying active, curious, and connected through learning, mentoring, and real-life experiences.
Over the coming years, PATH will:
Co‑create practical tools
to strengthen personal planning and decision‑making.
Connect individuals, workplaces, and communities
to advance age‑inclusive practices.
Pilot new learning pathways
that keep people active, engaged, and future‑oriented.
Explore what “healthy ageing” truly means
in a changing labour market and society.
PATH is more than a project
